Botox VIP Club Membership Agreement

Membership Terms & Conditions

1. Minimum Commitment. This membership requires a minimum commitment of six (6) consecutive months. Monthly billing continues for the full 6-month term regardless of service usage.

2. Automatic Renewal. After the initial 6-month period, this membership automatically renews at the same rate until written cancellation notice is provided per Section 4.

3. Member Pricing. Active members receive exclusive per-unit rates: Botox at $7.50/unit (reg. $12), Daxxify at $6.00/unit (reg. $10), and Dysport at $3.00/unit (reg. $6). Rates are subject to change with 30 days' written notice.

4. Cancellation Policy. After the 6-month term, the member may cancel by providing written notice at least thirty (30) days before the next billing date to admin@soomedspa.com.

5. Monthly Billing Authorization. SOO Med Spa will charge the authorized payment method on the same calendar day each month (or annually for the Annual plan). It is the member's responsibility to keep payment information current.

6. Non-Transferable Benefits. Membership benefits are personal and non-transferable. Discounts may not be combined with other promotions or packages unless expressly authorized in writing.

7. No Refunds. Membership fees are non-refundable. Consultation fees ($35) are credited toward treatments on the same day.

8. Failed Payments. If a charge is declined, SOO Med Spa will attempt collection up to two (2) additional times. If payment is not received within five (5) business days, membership benefits will be suspended.

9. Governing Law. This agreement is governed by the laws of the State of Texas.

Billing Authorization

I authorize SOO Med Spa to charge my payment method on my billing date for the membership fee of the plan I select, beginning on enrollment and continuing until cancelled per the terms above.

6-Month Minimum CommitmentI agree to a minimum of 6 consecutive monthly payments. I may not cancel during this initial period.

Automatic RenewalAfter the initial 6-month period, my membership auto-renews at the same rate until I provide 30 days' written cancellation notice.

30-Day Cancellation NoticeTo cancel after the 6-month term, I must notify Soo Med Spa in writing at least 30 days before my next billing date.

No Combining DiscountsMy member pricing cannot be combined with any other discounts, coupons, or promotional offers unless expressly authorized by Soo Med Spa management.
