Membership Enrollment Agreement & Billing Authorization

Membership Terms & Conditions
1. Minimum Commitment. This membership requires a minimum commitment of six (6) consecutive months. The member may not cancel, pause, or terminate this membership during the initial 6-month period. Monthly billing continues for the full 6-month term regardless of usage.
2. Automatic Renewal. After the initial 6-month period, this membership automatically renews month-to-month at the same rate until written cancellation notice is provided per Section 4.
3. Monthly Billing. SOO Med Spa will charge the authorized payment method on the same calendar day each month. It is the member's responsibility to keep payment information current.
4. Cancellation Policy. After the 6-month term, the member may cancel by providing written notice at least thirty (30) days before the next billing date. Written notice may be submitted in person, by email to admin@soomedspa.com, or via the member portal.
5. Missed Month Rollover. If a member misses their monthly appointment, the unused treatment credit carries over to the following month. The member may use both months' combined credit value in a single visit that following month. Unused credits do not accumulate beyond one missed month.
6. Membership Pause. After the initial 6-month term, members may request to pause for up to two (2) consecutive months per calendar year with five (5) business days' written notice before the next billing date.
7. Non-Transferable Benefits. Membership benefits are personal and non-transferable. Discounts and complimentary services may not be combined with other promotions or packages unless expressly authorized in writing.
8. No Refunds. Monthly fees are non-refundable. Unused monthly treatments, discounts, or credits are forfeited upon cancellation or expiration except where a carry-over has been applied per Section 5.
9. Failed Payments. If a charge is declined, SOO Med Spa will attempt collection up to two (2) additional times. If payment is not received within five (5) business days, membership benefits will be suspended.
10. Appointment & No-Show Policy. A minimum of 24 hours' notice is required to reschedule without penalty. Same-day cancellations and no-shows may result in forfeiture of that month's benefit.
11. Price Adjustments. SOO Med Spa may adjust membership pricing with thirty (30) days' written notice to the member's email on file.
12. Treatment Exclusions. Membership discounts do not apply to packages, promotional bundles, or items excluded at management's discretion.
13. Termination by SOO Med Spa. SOO Med Spa may terminate a membership for non-payment, abuse of benefits, or violation of clinic policies. Pre-paid fees beyond the current billing period will be refunded on a prorated basis.
14. Governing Law. This agreement is governed by the laws of the State of Texas. Disputes shall be resolved in Harris or Fort Bend County, Texas.
Monthly Billing Authorization
I authorize SOO Med Spa to charge my payment method on my monthly billing date for the membership fee of the plan I select, beginning on enrollment and continuing until cancelled per the terms above. It is my responsibility to keep payment information current.
